We have used the OT/PT CERT in Georgia although many school districts, including mine, have discontinued using it. It appears that you based this checklist on that document, is that right? Are speech-language services always a related service in your state? In Georgia, speech-language services are always provided through an eligibility, we are rarely a related service here. I could see that some of these questions may be helpful in determining if our services are warranted as a related service, but not useful in determining eligibility. When determining eligibility, there are only three questions: Is there a disability?, Is there an educational impact?, and Does the student require speech-language services in order to make progress in his/her curriculum? We would not consider factors such as student motivation, number of years student has received speech-language services or attendance in those decisions.
